Paragon Wound Care is an advanced mobile and outpatient wound-management practice dedicated to delivering evidence-based treatment—often within 24 hours of referral. Led by an interdisciplinary team of nurse practitioners, physicians, and nursing specialists, we partner with ALFs, and home-health agencies to reduce hospital readmissions and improve healing outcomes across Northeast Florida.
Position Summary
The Wound Care Nurse Practitioner functions as a primary clinical provider, managing a diverse caseload of chronic and complex wounds in facility, clinic, and home settings. You will collaborate closely with the Medical Director, Director of Clinical Operations, and bedside staff to ensure high-quality, guideline-driven care.
Key Responsibilities
- Perform comprehensive wound assessments, formulate evidence-based treatment plans, and implement advanced therapies (e.g., debridement, NPWT, cellular/tissue products).
- Order and interpret diagnostics; prescribe medications and durable medical equipment per state scope and company protocols.
- Document encounters in the EMR with accurate coding (ICD-10/CPT) to support optimal reimbursement and compliance.
- Educate patients, caregivers, and facility staff on wound-healing principles, prevention, and after-care.
- Participate in interdisciplinary rounds and quality-improvement initiatives; track outcomes and contribute to research/education projects.
- Maintain licensure, certifications, and continuing-education requirements; comply with HIPAA, OSHA, and infection-control standards.
Required Qualifications
- Active Florida APRN license & national board certification.
- Minimum 2 years of NP experience—wound care, dermatology, surgical, or acute-care background strongly preferred, but we are willing to train the right candidate.
- BLS (current).
- Strong procedural skills (sharp/surgical debridement, biopsies, graft application) or willingness to train.
- Valid driver’s license, reliable transportation, and proof of auto insurance.
- Excellent interpersonal and EMR documentation skills.
