Primary Care Nurse Practitioner (NP) – Long-Term Care
📍 Ohio | Full-Time | On-Site | Monday–Friday (8:00 AM–5:00 PM EST)
We are seeking a compassionate and clinically strong Primary Care Nurse Practitioner to provide comprehensive medical care to residents in skilled nursing and long-term care facilities . This role focuses on primary, preventive, and chronic care management for medically complex patients in a collaborative, interdisciplinary environment.
Responsibilities
- Provide primary and preventive care to long-term care residents in skilled nursing facilities
- Evaluate and manage residents experiencing acute changes in condition
- Perform advanced care planning and chronic care management
- Prescribe medications, including controlled substances, in accordance with Ohio law
- Order, review, and follow up on diagnostic tests and treatment plans
- Communicate regularly with nursing home staff regarding patient status and care plans
- Collaborate closely with the primary care physician and interdisciplinary care team
- Ensure accurate and timely documentation within the EMR
Required Qualifications
- Current, unrestricted Nurse Practitioner license in the state of Ohio
- Board Certification (Adult, Family, or Geriatric NP)
- Active DEA license
- Clinical background in adult, family, or geriatric medicine
- Prior experience working in a long-term care or skilled nursing setting
- Proficiency with electronic medical records (EMR)
- Ability to manage patients with advanced illness and multiple chronic conditions
- Strong teamwork skills within an interdisciplinary care model
Preferred Qualifications
- Strong clinical reasoning and problem-solving skills
- Ability to work independently while recognizing when escalation is needed
- High level of risk assessment and patient safety awareness
- Experience managing complex or high-acuity patient populations
Schedule
- Monday–Friday
- 8:00 AM–5:00 PM EST
- 5 days per week
- On-site position
