Job Description

Primary Care Nurse Practitioner (NP) – Long-Term Care

📍 Ohio | Full-Time | On-Site | Monday–Friday (8:00 AM–5:00 PM EST)

We are seeking a compassionate and clinically strong Primary Care Nurse Practitioner to provide comprehensive medical care to residents in skilled nursing and long-term care facilities . This role focuses on primary, preventive, and chronic care management for medically complex patients in a collaborative, interdisciplinary environment.

Responsibilities

  • Provide primary and preventive care to long-term care residents in skilled nursing facilities
  • Evaluate and manage residents experiencing acute changes in condition
  • Perform advanced care planning and chronic care management
  • Prescribe medications, including controlled substances, in accordance with Ohio law
  • Order, review, and follow up on diagnostic tests and treatment plans
  • Communicate regularly with nursing home staff regarding patient status and care plans
  • Collaborate closely with the primary care physician and interdisciplinary care team
  • Ensure accurate and timely documentation within the EMR

Required Qualifications

  • Current, unrestricted Nurse Practitioner license in the state of Ohio
  • Board Certification (Adult, Family, or Geriatric NP)
  • Active DEA license
  • Clinical background in adult, family, or geriatric medicine
  • Prior experience working in a long-term care or skilled nursing setting
  • Proficiency with electronic medical records (EMR)
  • Ability to manage patients with advanced illness and multiple chronic conditions
  • Strong teamwork skills within an interdisciplinary care model

Preferred Qualifications

  • Strong clinical reasoning and problem-solving skills
  • Ability to work independently while recognizing when escalation is needed
  • High level of risk assessment and patient safety awareness
  • Experience managing complex or high-acuity patient populations

Schedule

  • Monday–Friday
  • 8:00 AM–5:00 PM EST
  • 5 days per week
  • On-site position
